CHN offers a wide array of healthcare services encompassing addiction services, behavioral health, dental care, eye care, foot care, geriatric care, LGBTQ+ services, pediatrics, pharmacy, PrEP and PEP, primary care, wellness and nutrition, women’s health, sexual health, veterans healthcare, and a refugee health program. The organization operates a fleet of mobile health vans that serve underserved areas in Manhattan, Queens, the Bronx, and Brooklyn. Specialized care for older adults includes memory wellness assessment, emotional well-being, fall prevention strategies, bladder health, and bone health. CHN collaborates with the Institute of Community Living to provide a food pantry offering free groceries to community residents. In partnership with Mount Sinai Health System and NYC Health + Hospitals/Elmhurst and Queens Hospitals, CHN runs a Family Medicine Residency Program. The Medical Mobile Van delivers low-cost primary care, PrEP and PEP services, HIV testing and treatment, STI testing and treatment, and other sexual health services. The Health Home Program ensures members access necessary medical, mental health, and substance use treatment services. Dental services range from teeth cleaning to dental surgeries, including children’s oral care. Existing patients with urgent health problems can walk in without prior scheduling. A patient portal allows access to medical records, communication with CHN staff, and viewing lab results. CHN offers various birth control options at low or no cost and has provided PrEP services since 2012, covered by Medicaid and most private insurance plans. The Transgender Family Program supports trans/gender non-conforming communities and improves HIV prevention and care. Telehealth visits are available for remote consultations. CHN operates 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, and offers free home delivery of prescriptions through its in-house pharmacy at Long Island City and Jamaica Health Centers. Employees benefit from educational loans and assistance, with loans forgiven up to 100% after two years of service, and participation in the Public Service Loan Forgiveness Program. The comprehensive benefits program for employees includes health insurance, life insurance, and long-term disability. Women’s services include care for infertility, breast cancer tests, cervical cancer tests, diabetes tests, family planning services, and more.
