Camden Coalition of Healthcare Providers

Camden Coalition of Healthcare Providers is dedicated to improving care for patients with complex health and social needs. The organization hosts an annual conference for the complex care field and operates a consumer voices bureau called Amplify. Through their Learning Center, they offer a Complex Care Certificate and provide a variety of resources, including toolkits, briefs, reports, journal articles, and case studies. The Coalition engages in data analysis and integration to enhance care management and focuses on strengthening ecosystems of care. They conduct training in person-centered models and frameworks, participate in public policy efforts, and lead the Regional Health Hub work. Additionally, they develop and implement programming to support patients and utilize an integrated data system that merges individual-level data from health, criminal justice, and other social services.
